Residence statement
In accordance with the Shanghai Municipal Education Commission policies, Dulwich College Shanghai Minhang requires that each student live with at least one parent or guardian.
By signing below, I confirm that I/we, the parent(s) or guardian(s), will live in Shanghai and the student will reside with me/us. I/we confirm that the student applicant in my/our care has legal Shanghai residency status, and I/we will provide copies of my/our resident permit(s) and Shanghai working permit(s), including the student’s residency permit, no later than one month before the enrolment date. I acknowledge that should I/we be unable to provide proof of legal residency, Dulwich College Shanghai Minhang reserves the right to ask the child in my care to leave the school and all tuition fees paid will be forfeited.

Parent/Guardian Agreement

Submitting this application signifies agreement with the following:

1. The parent(s)/guardian(s) and the student applicant will abide by Dulwich College Shanghai Minhang policies and procedures.
2. The parent(s)/guardian(s) understand and agree that the student applicant may undergo academic or diagnostic testing.
3. The parent(s)/guardian(s) give Dulwich College Shanghai Minhang permission to contact the applicant’s previous schools to obtain information relevant to the application.
4. Dulwich College Shanghai Minhang has the right to keep all the information submitted with the application.
5. The parent(s)/guardian(s) agree to pay all fees and tuition according Dulwich College Shanghai Minhang’s Admissions policy.
6. The parent(s)/guardian(s) agree to inform the College if any information provided in this application form changes.
7. By signing this application I/we verify that the information provided is accurate and complete. I/We understand that failure to do so is grounds for nullification of our child’s enrolment at Dulwich College Shanghai Minhang.
8. I/We, the parent(s)/guardian(s), have read, have understood and agree to abide by the College’s Admissions policy.
9. I/We, the parent(s)/guardian(s), have read, have understood and accept the Terms and Conditions Governing Enrolment and Admission to Dulwich College Shanghai Minhang.
10. If this form has been signed/submitted by only one parent or guardian then that parent represents and warrants that she or he has the full and irrevocable authority from the other parent or guardian to make decisions, communicate, give instructions and take actions in respect of the student, and the College shall not be obliged to obtain the consent of both parents or guardians.
